Before Your Initial Visit

Please take the time to fill out the appropriate Intake Form available below. You may either bring it to the appointment with you or fax it to the clinic ahead of time. If you prefer, you can also complete an Intake Form in our clinic but, please arrive 15 minutes before your first appointment to fill it out.

Initial Visit

Your initial visit will be an hour in length. During this time, we will discuss your health concern(s) and medical history in depth. This allows us to get a clear picture of what is going on and find the root cause of your concern(s). During your fist appointment, we may also do a basic physical exam. If lab testing is required, we will perform it then or arrange for it at this time.

What should I bring to my Initial Visit?
Filled out Intake form (if you haven’t already faxed it into the clinic)
Any recent lab results (within 6 months) that you may have had done

Return Visits

Follow-up visits are typically 30 minutes in length. During these visits, we will implement and tailor a treatment plan specific to you and your goals. We may also review any lab results obtained.
